Modernizing a legacy EHR and practice management platform without disrupting live clinical care

Reengineered an aging EHR and practice management platform end-to-end, frontend, backend, data workflows, and healthcare integrations, modernizing scheduling, clinical documentation, e-prescribing, and interoperability while preserving the clinical and practice workflows already running in production.
The Challenge
Legend EHR's platform spanned an aging frontend, backend, database workflows, EHR/PMS modules, and multiple healthcare integrations. The legacy system already supported core EHR and practice-management operations, but its architecture and workflows needed to evolve to improve efficiency, speed, reliability, and cost, modernize the user experience, and introduce workflows that could meet growing demand, all without disrupting the healthcare operations already running on the platform.
We reengineered the platform end-to-end, frontend, backend, data workflows, EHR/PMS modules, and integrations, while preserving the critical business logic already running in production. On the clinical side, we built scheduling and provider availability, appointment reminders, and enterprise EHR workflows covering patient medical history, progress notes, immunizations, medication reconciliation, registries, and electronic note signing, with customizable templates for streamlined clinical documentation. E-prescribing workflows connect prescribers to pharmacies for medication management, and HL7 and FHIR interfaces support structured exchange of clinical and patient data with external healthcare systems. On the practice management side, we modernized patient demographics, copays and balance payments, referrals, user roles, and reporting, integrated Trizetto for insurance eligibility verification, connected pharmacy workflows into the broader prescription process, and integrated BridgePay for healthcare payment processing. Security, auditability, monitoring, and controlled access were designed into the architecture throughout to keep the platform aligned with U.S. healthcare requirements.
